By signing this document, I hereby acknowledge and agree to the following terms and conditions while participating in dance
classes, workshops, and events at Ballet Folklorico de Dallas Ft. Worth (Ballet Folklorico DFW/ Folklorico DFW/ BFDFW)
I hereby grant Ballet Folklorico DFW and its representatives the right to take photographs and video footage of me, my dancer, and my family during my participation in various dance activities and events organized by Ballet Folklorico DFW.
I understand that the photographs and videos may be used for various purposes, including but not limited to, promotional materials, advertising, social media, website content, and other marketing efforts by Ballet Folklorico DFW. These materials may be distributed and displayed to the public through various channels.
I authorize the use of such media for any legitimate purpose without compensation to me, my dancer and my family. I also grant Ballet Folklorico DFW the right to edit, crop, retouch, or otherwise modify the photographs and videos at their discretion, without any further approval from me.
I acknowledge that the photographs and videos taken during my participation and that of my family at Ballet Folklorico DFW are the property of Ballet Folklorico DFW, and I waive any rights to inspect or approve the finished product that may be used in connection with my name and likeness and that of my dancer and family. This includes photographs and videos shared with Ballet Folklorico DFW.
I release Ballet Folklorico DFW, its employees, representatives, and any third parties acting under its authority, from any liability or claims whatsoever in connection with the use of the photographs and videos, including any claims for defamation, invasion of privacy, or infringement of moral rights. This photo release statement shall be binding upon me, my heirs, legal representatives, and assigns.
I affirm that I am over 18 years of age, or that I am the parent or legal guardian of the named participant and have the authority to grant this release on their behalf. I have carefully read and understood the terms of this photo release statement before signing it.
